If you’re pregnant or breastfeeding, you’ve probably been told to take folic acid. It’s one of those things that gets mentioned at every appointment, printed on every prenatal vitamin bottle and repeated so often that we just accept it as gospel. But here’s what your doctor probably hasn’t told you: folic acid and folate are not the same thing. And for many women, that difference matters enormously.

I’m going to walk you through what I’ve learned about this, why it’s important and what you might want to consider for yourself and your baby. Because when it comes to something as critical as pregnancy nutrition, you deserve to know the full story.

What’s the Difference Between Folic Acid and Folate?

Let’s start with the basics.

Folate is the naturally occurring form of vitamin B9. It’s found in foods like leafy greens and legumes and lentils and citrus fruits. Your body recognises it, knows what to do with it, and can use it relatively easily.

Folic acid is the synthetic form. It’s what you’ll find in most supplements and fortified foods. It’s cheaper to produce and more stable, which is why it’s everywhere. But here’s the catch: your body has to convert folic acid into the active form of folate (called methylfolate or 5-MTHF) before it can actually use it. For some women, that conversion happens just fine. For others, it doesn’t. And that’s where things get complicated.

The MTHFR Gene: Why Some Bodies Struggle

There’s a gene called MTHFR (methylenetetrahydrofolate reductase, if you want the full mouthful) that’s responsible for converting folic acid into the usable form your body needs.
